Paperwork Required To Participate
One of the following items must be completed, and submitted to the Sanger Warrior Athletics Department (in the Washington Athletics Office) for your student to participate in sports:
- Online Athletics Registration Receipt, and Current Physical Exam (dated/stamped)
- Sanger Warrior Athletics Clearance Form (via QR Code), and Current Physical Exam (dated/stamped)
The completion of your forms verifies that you understand all requirements to participate in Sanger Warrior Athletics. Please keep a copy of all documents for your records.
